And don't forget to first try...Tessellation (which smooths and slightly increases the mesh size of the chosen item) for the clothing in the Scene List. Then Subdivision (which smooths and slightly decreases the size of the mesh) of the chosen item.
Then under the Modify Tab there is Conform/Calculate Collision of the clothing item, also there is an Increase Size feature under Conform and then under Cloth Layer Settings you can Auto Hide mesh under the chosen item.
Also you can alter the items mesh with the Edit Mesh feature by pushing or pulling or moving the point and polys.
You shouldn't really have to paint out anything but if so that would be a last resort in most cases.
