Here is a new problem: I made some custom shoes using the correct method on a neutral base character, used the hide body mesh tool, and saved the shoes to the library. When I apply the shoes to a base character they fit correctly.
When I alter the base character with morph sliders, especially sliders that effect the hip width, the shoes no longer fit correctly. The shoes move away from center (see my attached sample). I can, of course, edit the mesh, select the shoes and slide them until they fit correctly, but this should work properly.

As a follow up, please note that I am not saying that I am modeling the character with the shoes on. I model the character with morph sliders and then apply the saved custom shoes from the library. When I apply the shoes they are misaligned.

After playing around with the morph sliders, I found that it was the hip distance slider that was causing this issue. I had the legs very close together - a more stylized character - and it was obviously too close for CC3 to apply the shoes correctly. By moving the hips (inner leg distance) out just a little, the shoes went back to fitting correctly. Problem solved.
